The plugin in that post is already nulled to "Elite" version but the advanced fields are still disabled for me.

Some of the so called "Fancy Fields" are still disabled: Signature, Custom Captcha, Likert Scale and Net Promoter Score. BTW, the version you poste is 1.6.8.1 while the version I installed previously was 1.6.9
