When you are having 500 error, put your website on debug mode so you can see exactly what is the problem. I have the problem wehn I try to open OTP credentials too, I just went to the database I found 2 unwanted da row in the OTP field and I just deleted them and everything start to work fine , I even try Zender OTP and it works also
