No one directly deploy on production, unless they fix or improve the code, I never seen a code which work out of the box in production, you must customize as per your choice, I am not tell you wrong, but codecanyon developer don't have huge team with QA, usually single person doing everything, so he/she could make some blunder, that's why I never trust envato codes at all, unless I review my self and fix the code as per my need.
True. But i have purchased lot of codes and most of them work flawlessly (maybe sometimes you need to contact support) . This developer is just ignoring the bugs and selling it. This must be by far the worst code ever.
Depend on the codebase size, usually big codebase which involved apps can be messy
